Chronic Kidney Disease is a personal challenge... and a public health crisis.

Over 15% of the US population is living with CKD, with more than 90% of people being unaware of their diagnosis. Risk factors include diabetes, hypertension, heart disease, and obesity. These conditions are epidemic among medically underserved and disadvantaged populations, with African-Americans (3.5x more likely to develop kidney failure), Latinos and Native Americans particularly afflicted.

To address ESKD adequately requires that we forcefully address the root causes of health disparities and inequity both inside and outside the health care system.

We do that by turning the system inside-out, bringing services to the patient on their terms. Key to our success is our partnerships with local nephrologists, whom we seek to empower with accretive financial arrangements and keener insight into their patients’ rapidly evolving condition. Duo Health medical groups use advanced analytics to identify patients early and our machine learning tools better predict risk of kidney function decline.

We design a care plan to be executed by a small team, each member of whom our patients are on a first-name basis. If and when a transplant (13 patients die every day waiting for a transplant) or dialysis (100,000 new hemodialysis starts each year) becomes necessary, we guide and support them through this and the transitions to come.
