Duo is a new type of mobile medical group in a contracted relationship with your insurer that takes responsibility for all aspects of your care.
Your Duo team is purpose-built around you and your needs and is comprised of physicians, nurses, pharmacists and other allied professionals all working in coordination with your existing doctors. We bring our first-class treatment to your home or any other site of your choice. There are no additional fees to you for joining Duo.
Your team works with a small number of patients like you who live in your community. You’ll work with this same team throughout your Duo experience. We use our advanced technology to anticipate what is coming next for you in your health journey so we can make a plan together. And we make it easy to talk to your team. No phone trees. No run-around. We might not be able to solve a problem that moment. But we won’t leave your side until the issue is resolved. That is our promise to you.
As a full-service medical group, we take on all aspects of your care, even if it is not related to your kidney disease. This model ensures that we can and will spend as much time with you as is necessary to ensure your better health.

Yes. And Duo will help connect the dots between all off your doctors including those you may see in the hospital, to put everyone to the same page.
Does this change my Medicare coverage?
No. As a medical group, Duo is contracted with your managed care plan who is working to organize and finance your care alongside us.
Just think of us as the clinical team who will meet you anywhere and is holding all the pieces together.
How do I use Duo?
When you choose to utilize Duo services, we build a team around you, based on your health status, your living arrangements, your neighborhood, and your preferences. That team stays with you through every step of your health journey. They are available to you 24/7/365. Call us first.
Your Duo navigator can help you get the most out of your healthcare experience. Your health plan can help answer questions about your coverage and benefits.
For example: our team cannot change the classification of a medication within your plan but we can recommend alternatives and generic options.
What do you need to know?
Our job is to know everything you want to share. When you join Duo your team will review your medical records, come to your home or a place of your choosing, create a care plan for you and learn everything important to you: from your grandkids’ names, your favorite sports team, your biggest goal for the year, and so on .
Have you talked to my doctor?
Part of our job is to coordinate with your doctor and our team so you don’t have to. Our multidisciplinary care teams are comprised of everyone you need to keep you healthy.
